Like the stance on the 04', spacers? Any rolling?

Did you replace the coil pack? Also could be a sticking or burnt valve in the head. If it isn't misfiring, possibly an issue with cam or crank sensor at high rpm. Does it happen if you slowly rev above 5k, low throttle input?

Adjusting the pedal? Easy as it gets!

The crazier colors seem to go better with more thoroughly done exteriors IMO. Too much for stocker looking cars.

To my knowledge they're just a self-test, but I could see how a little exercise for the needles might correct a little deadspot, or something along those lines.

There are ways to test a PCM to determine if it's bad, it's definitely not a part I would just swap out and cross your fingers that it works. ^Heh I didn't realize the dates! Answers it for others who may have that issue though. I could see them smoking if there was a break-in coating on the pads or rotors, or if oil/grease got on the contact surfaces of either.

Front strut mounts are indeed common to make noise. Might be tough to narrow it down without pulling the strut assembly out though. If all the bushings, axles, ball joints check out, it's a good bet it's the mounts, especially at that mileage. When struts are replaced the mounts are typically reused...
