Take pictures of everything as you take it apart, especially if you have rear drum brakes. Biggest thing I see people forget on their first time is to pump the pedal a few times after everything is done.

You probably have excessive play in your drivetrain mounts to get a jerking like that to occur. One very basic way to check your motor mounts and transmission mount is to jack the vehicle up, support it on jack stands and make sure its safe to get under. Then have someone start the vehicle and shift...
