Well its either the bulb or the ballast. A loose connection could cause it as well. Try unplugging your bulbs from your ballasts and swaping them. If the other side is then dark its the bulb, otherwise its the ballast or a loose connection.

good luck trying to match it up. Even though they might advertise the same kelvin rating, chances are the color will be different.

Yes any of the bulbs off of ebay will fit. You can get a pair for 34.99 shipped usually. Just make sure you look closely in the pictures to be sure that the connectors on the bulb match up with the connectors on your bulbs. Almost all aftermarket kits use the same harnesses and connectors though.
I wouldn't buy anything over 6,000K personally. I havfe 6,000ks in my stock headlights and 4300ks in my retrofits. Anything above 6,000k and you are drastically comprimising your ability to see at night. The 6,000ks have a very slight purple tint to them, but are a sheer beautiful true white. 5,000ks are a very nice white as well.
Its probably not the ballast, and just that one bulb has worn out faster than the other. Make sure you replace them in pairs, but keep your old ones in case you need to change out a blown one sometime soon.
