SRT-4 Hurst Shifter Fitment Issues

The How-to and subsequent discussion was helpful, but since the pictures are no longer available it's hard to follow. Basically, I'm mounting the Hurst assembly onto the OEM SRT shifter cradle. 1) When I keep the black spacer between the assembly and the cradle I get locked out of 5th and Reverse. I read the solution was to just remove the spacer, but it create my second problem.

2) When the spacer is removed and the assembly is bolted straight to the cradle, it does no sit flush. This results from a small plastic tab which protrudes from the pivot surround (not sure if this is the right term). Normally this tab would sit inside the space which is in the spacer, but see problem 1 why that isn't working.
